4.12.1: Tap on the sidebar and select "Health Status" tab to get the health declaration form.

4.12.2: Choose the "Health Declaration" from the selection.

4.12.3: See the list of questions. Answer the questions YES Or NO.

4.12.4 : Read the declaration form and Tick in the space provided.

4.12.5: Click “Submit” Button.
